Who qualifies

Who can get healthcare allowance in 2026

The Belastingdienst lists 4 conditions for 2026: you are 18 or older, you have a Dutch health insurance policy, and your income and your assets are below the limits (Belastingdienst).

  • Dutch basic insurance: any basic policy from a Dutch insurer counts; supplementary cover is not needed. A visitors’ policy or a voluntary policy for foreign students does not count (Belastingdienst).
  • Lawful residence: EU, EEA and Swiss citizens need a valid residence status; others need a valid residence permit that gives a right to allowances (Belastingdienst).
  • Your partner: your allowance partner is your spouse or registered partner, or sometimes someone registered at your address (Belastingdienst). If your partner has no Dutch insurance, you get half the amount for 2 people, but their income still counts (Belastingdienst).
  • Students: if you only study here, you usually stay insured at home and cannot get it. A paid side job, or an internship paid at least the minimum wage, changes that (Belastingdienst).

Amounts

How much healthcare allowance you get in 2026

The Belastingdienst’s 2026 table pays up to €129 a month if you are single and up to €246 a month with an allowance partner, and less as your income rises (Belastingdienst).

The premium you pay your insurer does not change the amount.

Limits

The income and assets limits for 2026

For 2026 your income may not be higher than €40,857, or €51,142 together with your allowance partner, according to the Belastingdienst (Belastingdienst).

  • Which income counts: the toetsingsinkomen (assessment income). It is roughly your gross income for the year, usually a little higher, and never your net income (Belastingdienst). Holiday allowance counts, student finance does not (Belastingdienst). Our Dutch tax calculator and income tax guide help you estimate your year.
  • Assets on 1 January: only your assets on 1 January 2026 count: up to €146,011, or €184,633 together. Savings count, and so does a holiday home. Too much on 1 January means no allowance for all of 2026, even if your assets fall later (Belastingdienst).

Apply

How to apply for healthcare allowance, and the deadline

You apply yourself in Mijn toeslagen, and you usually get a letter within 5 weeks of applying (Belastingdienst).

  1. Register and take out Dutch health insuranceYou can apply once you are registered with your municipality and insured (Belastingdienst). Registering is also how you get your BSN number. Your insurer tells the Belastingdienst you are insured (Belastingdienst).
  2. Have your income readyEnter your expected income for the year, and your partner’s. Estimating a little too high lowers the chance you pay back later (Belastingdienst).
  3. Log in to Mijn toeslagenMost people use DigiD; EU residents can use the European login (Belastingdienst).
  4. Choose Zorgtoeslag, then AanvragenAnswer the questions and send. You usually see straight away how much you will get (Belastingdienst).

Deadline: healthcare allowance over 2025 can still be requested until 31 December 2026 (Belastingdienst). That page does not yet give the last date for 2026, so apply as soon as your insurance starts. Once you have applied, it continues the following year (Belastingdienst).

Changes

Reporting changes to your healthcare allowance

You must report changes to the Belastingdienst within 4 weeks, in Mijn toeslagen (Belastingdienst).

  • Income: a new job, a raise or more hours means a new estimate, and give a fresh one before each new year (Belastingdienst).
  • Assets too high on 1 January: report them within 4 weeks and the Belastingdienst stops the payments (Belastingdienst).
  • Your address: a move you report to your municipality does not need reporting again (Belastingdienst).

Paying back

Paying healthcare allowance back

After the year ends, the Belastingdienst makes a final calculation with your real income; if you file a tax return, it arrives within 6 months of your tax assessment (Belastingdienst). If your income was higher than your estimate, you pay back the difference (Belastingdienst).

  • Small amounts: for a final calculation received in 2026, you do not pay back €121 or less per allowance; a recalculation during the year is always reclaimed (Belastingdienst).
  • Payment plan: up to 24 months, with 4.3% interest since 1 January 2026 (Belastingdienst).
  • Objecting: to make a formal objection (bezwaar), you must do so within 6 weeks of the date of the letter (Belastingdienst). Missed it? You can still ask for a review of the decision (Belastingdienst).

Moving away

Leaving the Netherlands: what happens to your healthcare allowance

Without Dutch health insurance you have no right to healthcare allowance; stop it in Mijn toeslagen and you usually get a new calculation within 5 weeks (Belastingdienst).

  • Your address: give it to your municipality before you leave; it informs the Belastingdienst (Belastingdienst).
  • Keeping it abroad: sometimes possible, for example if you work for a Dutch employer or are insured through the CAK. The amount can then be lower (Belastingdienst).
  • Rent allowance stops: rent allowance is not paid while you live abroad (Belastingdienst).
  • From abroad: with DigiD or the European login you can still use Mijn toeslagen (Belastingdienst).

FAQ

Frequently asked questions

How much healthcare allowance will I get in 2026?

It depends on your income and on whether you have an allowance partner. In the Belastingdienst’s 2026 table, a single person with an income up to €29,500 gets €129 a month, falling to nothing at the €40,857 income limit. With a partner you get up to €246 a month together,.

Can expats and international students get healthcare allowance?

Yes, if you must take out Dutch basic health insurance and meet the other conditions. EU, EEA and Swiss citizens need a valid residence status; others need a valid residence permit that gives a right to allowances. Students who only study usually cannot get it.

When is healthcare allowance paid?

Every month, as an advance, paid before the month it is for. The Belastingdienst pays on 20 October 2026 for November and on 20 November 2026 for December. The final amount for the year is only fixed after the year ends.

Why do I have to pay healthcare allowance back?

Usually because your real income, or your partner’s, was higher than the estimate your allowance was based on, or your assets were too high on 1 January. For final calculations received in 2026, amounts of €121 or less per allowance are not reclaimed. Larger amounts can be paid back over up to 24 months.

Can I object to a healthcare allowance decision?

Yes. You must object within 6 weeks of the date on the letter, for example against a final calculation or a letter about paying back. If you miss that deadline, you can still ask the Belastingdienst to review its decision. If you disagree with the decision on your objection, you can appeal.

What happens to my healthcare allowance if I leave the Netherlands?

Without Dutch health insurance you have no right to it, so stop it in time in Mijn toeslagen to avoid paying back. Your municipality passes your new address on. You can sometimes keep it abroad, for example when you work for a Dutch employer.
